How to Apply
To complete the admissions application, please send a copy of each of the following forms to Fiona Diemer at the below address:
- Birth certificate
- Baptismal certificate (if applicable)
- Health forms
- Immunization form
- Most recent report card
- Most recent standardized test scores
- Grades Kindergarten, Pre-First, and 1:
- Grades 2 through 8: Teacher Referral Form
